What are F codes?
F codes are ICD-10 codes that describe mental health disorders. They can be found in a diagnostic guide called the International Classification of Diseases, Tenth Revision (ICD-10), which is published by the World Health Organization (WHO). The ICD-10 classifies and defines health conditions using standard diagnostic language.
In the United States, clinicians also use the Diagnostic and Statistical Manual of Mental Health Disorders, fifth edition (DSM-5), published by the American Psychiatric Association (APA). The DSM-5 also contains the relevant Dx coding information. For example, the Dx code for unspecified anxiety disorder would be listed as F41.9 in both the ICD-10 and DSM-5.
Using the appropriate ICD-10 codes is critical for clinicians to use as part of the assessment process and to communicate to insurance payers.
What is the Dx code for unspecified anxiety disorder?
Anxiety disorders are classified in the ICD-10 under the section “Anxiety, dissociative, stress-related, somatoform and other nonpsychotic mental disorders” (F40 to F48).
The Dx code for unspecified anxiety disorder (F41.9) is listed in the sub-section “Other anxiety disorders” (F41).
Code F41 should not be used for reimbursement purposes as there are codes below it that contain a greater level of detail, including:
- F41.0: Panic disorder [episodic paroxysmal anxiety]
- F41.1: Generalized anxiety disorder
- F41.3: Other mixed anxiety disorders
- F41.8: Other specified anxiety disorders
- F41.9: Anxiety disorder, unspecified
Dx code F41.9 is listed at number five in SimplePractice’s list of the 20 most-frequently billed ICD-10 codes.
If you’re looking for the ICD-10 code for anxiety and depression, you may want to use the code for “Other mixed anxiety disorders” (F41.3).
This code is used when both symptoms of anxiety and depression or another disorder are present, but neither is predominant to the extent of justifying a separate diagnosis.
F41.9 diagnostic criteria
The ICD-10 characterizes other anxiety disorders as “the manifestation of anxiety is the major symptom” but it is not limited to particular situations.
Other features include depression, obsessional symptoms, and phobias, but none are as prevalent as the main symptom of anxiety.
The DSM-5 provides more comprehensive F41.9 symptoms, including:
- Anxiety symptoms, including:
- Excessive worry that's difficult to control
- Worry and anxiety associated with symptoms of restlessness, being easily fatigued, difficulty concentrating, irritability, muscle tension, sleep disturbance
- The symptoms cause significant distress or impairment to social, occupational, or other important areas of functioning
- Does not meet the full criteria for any specific anxiety disorder or adjustment disorder, or adjustment disorder with mixed anxiety and depression
- Can be used when the clinician decides not to specify the reason criteria are not met for a specific anxiety disorder
- Is appropriate for situations where there is insufficient information to make a more specific diagnoses, such as in an emergency room setting
Differential diagnoses
While patients may meet some of the criteria for Dx code F41.9, there may be a more appropriate diagnosis, such as:
- Substance/medication induced anxiety disorder
- Social anxiety disorder
- Separation anxiety disorder
- Panic disorder
- Illness anxiety disorder and somatic symptom disorder
- Depressive, bipolar, and psychotic disorders
- Post-traumatic stress disorder and adjustment disorders
How to find the correct ICD-10 code
You can find Dx code F41.9 in digital and book formats of the ICD-10 and DSM-5, using the following links:
- The official ICD-10 from the WHO, under the section “Other anxiety disorders” (F41 to F41.9)
- ICD-10 Data, a searchable ICD-10 database
- DSM-5 in the textbook and digital version
It is worth noting that while the DSM-5 does contain accurate ICD-10 codes, they may not align with the most recent ICD version, and diagnostic criteria and other indicators may differ.
